U can try with antibiotics and conservative approach if symptoms reduced and collection reduced then no need of surgery. If clinically pus discharge present Or having fever and elevated blood counts then need surgery.

Hello 🙏 I understand your confusion. Your MRI shows a peri-anal abscess (pus collection) but no formed fistula tract yet. This is actually a good stage, because a fistula has not developed. Even when MRI says “no fistula,” surgery may still be required because pus does not resolve fully with medicines alone. If it is not drained, it can persist or later turn into a fistula. That is why your doctor has advised incision and drainage, not fistula surgery. No muscle involvement mentioned is a positive sign, and early drainage usually heals well with sitz baths and local care. Delaying treatment can increase future fistula risk. Please follow your surgeon’s advice. Wishing you a smooth recovery.

From the history mentioned in the Mri report, it seems you have a peri anal abscess. Though the Mri report suggests that there is no fistula tract, it shows there is fluid collection. That needs to be drained promptly. Otherwise there are high chances that it will develop into fistula in future. It is good that muscle planes are not involved now. A simple incision drainage if done now, the wound will heal by itself with post-drainage hygeine maintenance with sitz bath and medications.
Next Steps
general surgeon consultation and plan for drainage (based on the clinical examination)
