Gastric adenoca
Patient has Gastric adenoca antropyloric region 6×5×4 cm with few regional l.n involvement..presently persistent black vomitings due to severe gastric outlet obstruction and cannot tolerate liquids as well vomits out black water..want to consult if we should go for palliative resection n gastrojejunal bypass or just the bypass to relieve his vomiting n symptoms?...he also has h/o cardiac dis ,i.e, 90% LAD lesioned n grade 1 atheroma in arch for which he is on medications n no surgical intervention...he has h/o DM n HTN but presently he is showing hypoglycemic trends from 1 n half month..kindly suggest me with which procedure i shud proceed

In view of his cardiac condition, he would be a high risk for surgery. If it's possible, stenting would be a better choice followed by chemotherapy and management of cardiac issues followed by definitive surgery at a later date
