Facing pain & freaking painpassing stool

My baby who is soon going to be completing his 1st year is facing challenges during passing his tools from past three months initially it was very Severe but after consulting doctor there was bit of relief. Still it's not resolved completely, the initial stool is bit hard for my son to pass it gradually it becomes soft. But this process has terrified him a lot. Given lactulose but no use. Unable to find any improvement. Please suggest some good medicine. We are daily giving him oats porridge, cerelac is different varieties, apple or pear, enfamil for milk. Please suggest
