Dry and cracked penis

Good Evening. I am a 20 male. Since couple of weeks i am facing a problem related to my penis. The skin of my penis is dry and cracked and it gets bloody too. It is causing me a lot of pain and itchiness. A slight touch leads the dry skin to crack open and then it starts bleeding. I cant even wear an underwear or fit trousers because it starts to pain. Please help me in facing this problem.
