I noticed a large tender spot behind my right ear about a week and a half ago. It’s feels like a bruise. The bone behind the cartilage is sticking out prominently, but is still hard, not swollen. The mother said it is a bit red, but it could be from me touching it. I currently have a perforated eardrum in my right ear because of a hole that didn’t close after tubes surgery when I was a baby. Sometimes water gets in it when I shower, but I haven’t had ear infection-like pain.

You seem to be having enlargement and inflammation of post auricular lymph node probably secondary to ear infection or scalp infection. Mastoiditis is a unlikely diagnosis as it's not very common. You may need to take antibiotics and painkillers for few days
Next Steps
Consult nearby ENT specialist for confirmation of diagnosis and appropriate treatment if required
