Constipated baby 4 months old

Asked for Female, 34 Years

My baby is suffering from extreme constipation and is grunting badly and has rashes near anal area due to build pressure to pass motion. I gave neopeptine but it's not helping not Doctor suggested emty syrup 3ml twice a day pls can I give it to baby this small. He is on combination feed due to my milk supply being on lower side.

Pls show to any pediatrician as may be becoz of some rash or rectal injury child might be groaning... Usually constipation is unlikely for this age
