Doctor, I have chest pain for more than 3 years. The pain is mostly on the left side but sometimes moves around the ribs. It is not constant but comes and goes. ECG and Echo were normal. Endoscopy showed reflux. Pain sometimes increases when I lie down or after food, and sometimes it feels muscular or nerve-related. I feel pulling sensation occasionally. Medicines give partial relief but pain returns. I want to know if this is costochondritis, nerve pain, or gastric-related, and what tests/treatment you suggest."

Your findings suggest symptomatic hiatal hernia with reflux rather than cardiac or rib disease.
Persistent pain despite medicines may occur due to acid exposure or esophageal spasm. Continue PPIs, lifestyle modification, and avoid late meals.
If symptoms remain troublesome, surgical evaluation for laparoscopic fundoplication or hernia repair can be considered.
