Sir/madam i am having pregnancy of thirty six weeks and is having total bilirubin 150, sgot 220, sgpt 234, alkaline phosphate 30six. what is my present situation and what should i do

Dear Ma'am, Thank you for your query regarding, abnormal liver function tests with pregnancy at 36 weeks. At the outset,  I'd like to enquire why this test was ordered?  We're you having particular symptoms like yellowish discoloration of skin,  itching,  upper abdominal pain or as a part of investigation panel for gestational hypertension. This result indicates a rise in certain enzymes and byproducts associated with hepatic metabolism. Common causes for liver dysfunction in pregnancy include: 1. Gestational hypertension and HELLP syndrome. 2. Viral hepatitis 3. Acute fatty liver in pregnancy. 4. Gallstones 5. Obstetric Cholestasis. However, the presentation, investigation and further tests and management would differ for different pathology, thereby increasing the mortality and morbidity associated with this pregnancy. I would suggest an urgent review with your treating Obstetrician,  to help rule out the above disorders to optimise pregnancy outcome. It would be also good to start counting your baby's movements daily till your next visit.  More than 10 fetal movements in 2 hours would be considered normal. Sleeping in lateral position would help increase uterine blood flow to the baby. Do let me know if there are any further queries. Regards, Aditi
