Usually these closes after birth ,for preterm and in some term babies also some unable to close but we have to see shunting of blood also until unless the size is increasing or reversal of shunt occurs nothing to worry chances of closing spontaneously is more.

Wait and watch till 1 year, may close spontaneously.
Avoid exposure of cold, prevent from cold and cough.
Ensure adequate feeding for healthy wait gain.
Monitor size every 6 month with echo.
Immunization proper specially pneumococcal, meningococcal, H. Influenza vaccine.
