Anal Disease. Or someone else?

Hello doctors, I am 23 years old male and homosexual by nature! I had multiple partners before, used to have sex with my partners( I should mention this fir better understanding). I am having anal disease for years now,i have no idea exactly what disease it is. The symptoms are included : i feel pain in anal from inside mostly in right side sometimes left,i can't stand for hours it feels pain,have seen some spikes in my anal and sometimes thrush on my tongue it feels pain too.I am very narvous how to confess with doctors. Please suggest me, to whom should i consult for the treatment, General surgery or gastroenterologist or sexologist or someone else ? I have no idea , looking for the treatment urgently.
