Wisdom tooth extraction
I have cavity in right lower wisdom tooth and adjacent 7 molar. OPG attached along. Currently problem is sensitivity and food getting stuck and future complications. No pain currently. Dentists recommended wisdom tooth extraction and RCT of 7 no. But they said risk of Transient parasthesia due to wisdom tooth being near to nerve. Any solution to avoid risk of parasthesia like only RCT and can I delay extraction it if not pain right now?

As per the radiograph attached it seems like your impacted molar needs to be extracted followed by examination of the adjacent molar teeth.

Kindly get the CBCT done for cprrect interpretation of the mandibular nerve position and plan for the removal of impacted teeth at the earliest.

According to history, you have impacted wisdom tooth and deep cavity in 7 of right side. The best treatment option is extraction for 8 and RCT for 7 as advised to you. And wisdom tooth is near to nerve so there is chance of transient paresthesia which can be temporary and some times permanent also. Wisdom tooth is angulated so it required extraction in anyway. But before extraction go for cbct for 3D representation of your tooth position, so oral surgeon can plan your treatment accordingly to save nerve during extraction.

The radiographs reveals an impacted wisdom tooth i.e not completely erupted and positioned in a manner to cause food lodgement.
Extraction for the third molar would be advisable, but regarding the risk of paresthesia, it would be wise to comment only after an additional CBCT radiographs (3D X ray). As the present X ray could be a misleading one being a 2D X- ray.
Regarding treatment plan for 7 i.e the second molar we would want to comment only after a clinical check up.
