In a Grade 3 varicocele, the veins are visibly and palpably dilated, so it is important to distinguish between symptom control and correction of the underlying venous abnormality. At present, there is no good-quality clinical evidence that Ayurveda or Homeopathy can permanently close/reverse the dilated veins of a Grade 3 varicocele. They may potentially provide supportive relief of discomfort or improve general wellbeing, but they should not be presented as an alternative to definitive varicocele treatment when there is a clear indication for intervention. There is substantially better evidence for varicocele repair in appropriately selected patients. Current urological guidelines support considering repair particularly in men with a palpable clinical varicocele, infertility and abnormal semen parameters.

Management of Grade 3 varicocele varies from person to person. Under the guidance of nearby qualified   Ayurvedic surgeon/physician, Panchakarma therapies-such as Jalaukavacharana (leech therapy) and Basti (medicated enema)-can be effective. In Ayurveda, this condition is correlated with Siraj Granthi or Siraj Vriddhi& is managed using classical formulations like Triphala Guggulu, Varunadi Kwath, and Vriddhivadhika Vati to relieve symptoms
