Upper abdomen discomfort

I have been experiencing a persistent dull discomfort on the left side of my upper abdomen, just below the ribs, since around early September. The pain is not sharp, but feels like a constant pressure or heaviness, as if something is inside on that side. The discomfort becomes more noticeable when I sit for a long time, bend forward, or stay in one posture. Doctor suspected Costochondritis and Gilbert’s syndrome. Investigations: • USG Abdomen: Liver mildly enlarged (15.3 cm); early fatty change in pancreas • LFT: SGOT 21.8, SGPT 23.1 (normal) • Bilirubin: Total 2.1 (Direct 0.37, Indirect 1.73) • Amylase/Lipase: Normal • Lipid Profile: TC 182, LDL 116.9 (mild ↑), HDL 45 • TSH: 5.89 (mild ↑) • Vitamin D: 19 ng/ml (low) • HbA1c: 4.9% • Kidney, CBC, Urine: Normal Alcohol: Occasional(monthly now once/twice or even no) Smoking: occasional Current meds: Shelcal HD, Lumia 60K, Evion Suggest what more to do now and what it might be?

Review of the Complete blood count is warranted and depending on that a peripheral smear or reticulocyte count might be indicated. And assessment of the size of spleen on usg abdomen. This could be due to an enlarged spleen. Besides costochondritis could be the reason.

Could be a muculoskeletal pain due to Vitamin D deficiency. Continue the meds and finish the course. Repeat Vit D levels after 3 months
