TVS and D&C report difference.

My mother age 50 - got her TVs done for irregular and sometimes heavy/prolonged period for past 1 year. Her tvs scan showed severely thick endometrium measuring 3.1cm with multiple cystic focii. Doctor performed hysteroscopy D&C immediately within a week of the report. After the procedure doctor told tvs report was not accurate as they did not find anything suspicious in hysteroscopy and lining was also not severely thick. D&C report came out as hyperplasia or malignancy not seen. Disordered proliferative endometrium. My question is which report should we trust ? And is it possible to tell the thickness and suspicious areas inside uterine cavity with Hysteroscopy ? Really worried if we are missing out on something.
