Without x-ray how to determine whether to have fillings or root canal to the tooth? On what basis do we decide this?

Pain is main criteria !! Sensitivity is different from pain.. Pls make a note of it !! Night pain also suggests a need for RCT..

It depends on the symptoms like pain on percussion, sensitivity, increased pain at night, swelling. If all these symptoms are there then we have to go for root canal otherwise we go for fillings.
Next Steps
Observe if pain is increasing at night and is going unbearable or pain is only while chewing.
