Hi sir...    Your picture shows you have cavity  in your  upper 1 st molar teeth that reaches pulp and the crown structure is almost broken.....   That condition is already chronic and it rarely produce pain...if you leave  the cavity as such the infection may also affects periapical bone also... X-ray (IOPA) of the involved teeth should be done ..followed by ROOT CANAL TREATMENT of the involved teeth and capping( crowning)...
Next Steps
kindly visit nearby dentist for furthur conaultation and management...

Visit a good dentist at the earliest. We can now save this tooth by doing Root canal treatment and crowning (cap). But if u delay further it might have to be extracted. And keeping a decayed tooth for long might be harmful for other good teeth which may also get caries due to d bacteria present in this tooth

Hi.... It’s a tooth which has been decayed for a long time but you have not taken care of it. It’s a chronic condition now so you are not having pain now. This tooth requires treatment otherwise the infection can spread in the periapical( around the root) or can go to the bone or adjacent tooth.
Next Steps
Get an X-ray done. Visit a dentist

Very old (chronic dental cavities) seldom pain But they are centre for bacterial growth. In due course of time adjacent teeth will get effected. It may never pain now... So don't wait for pain
Next Steps

It is quite a big cavity. Get an X-ray done. If possible a filling or otherwise it may require a root canal treatment.
