Teeth plaque and tartar, bad breath

I eat a minimal food with very less junk. I was suffering from gum bleeding, but after using herbal tooth paste, it is fine, plaque and tartar is getting accumulated in front teeth and lower teeth. I am brushing the teeth twice a day. Kindly advice.
