It is possible for a small swelling and intermittent pain near a surgical site (like the area of your cholecystectomy stitches) to be related to the underlying muscle or other tissue. However, it’s important to consider several potential causes to understand the situation better: Possible Causes of Swelling and Pain Near Stitches: Muscle Strain or Scar Tissue Irritation: The surgical area can develop scar tissue or adhesions, which might cause discomfort, especially if there is strain on the muscle or surrounding tissue. Hernia (Incisional Hernia): A common complication after abdominal surgeries is an incisional hernia, where part of the abdominal content (like fat or intestines) pushes through the weakened muscle layer near the surgical site. This can present as a swelling that might vary in size and cause occasional pain. Fatty Tissue or Lipoma: Sometimes, small benign growths like lipomas (fatty lumps) can form under the skin and be mistaken for swelling. Localized Infection or Inflammation: Even years after surgery, minor infections or inflammatory reactions can occur near the stitches, especially if there's scar tissue irritation. Muscle Weakness or Bulge: If the surgeon mentioned it’s muscle, it could be a localized bulge due to muscle weakness or thinning, possibly caused by the incision and subsequent healing process. What You Can Do: Observation: Monitor the swelling for changes in size, color, or tenderness, especially when you move, strain, or cough. A hernia may become more prominent with these activities. Imaging Tests: Ask your surgeon if an ultrasound or CT scan can be done to rule out a hernia or any other underlying issue. Avoid Strain: Minimize heavy lifting, coughing, or straining that could increase pressure in the area. When to Seek Immediate Attention: If the swelling becomes hard, increases in size, or is associated with severe pain. If you notice redness, warmth, or pus, which could indicate infection. If there are symptoms of bowel obstruction, like nausea, vomiting, or constipation, which can occur with an incarcerated hernia. While muscle irritation can sometimes explain the symptoms, further evaluation (like imaging) may help ensure no other condition is causing the issue.
