Sleep disorder

I don't remember having a decent sleep since 2015. When I sleep, I either see dreams that keeps my "thoughts" running or either have nightmares that won't let me sleep back. Have frequent headaches, experience fatigue most of the days. What could be the problem? Is there any? If yes, then how do I solve it?
