Red spot on scrotum on rash

I have rashes on both sides of scrotum. On those rashes i have red spots that itches. In the middle tge scrotum is darkened. Pennis foreskin unable to retract. Pennis tip red and pain at tip when touched.Doctor gave medicine like moisturizer, onbact ebernet and nadibact. Skin become red like polythene. Please help me
