You have to stop smoking ABSOLUTELY.
Even if MPN panel was negative, this could be primary polycythemia.
Erythropoietin is normal which suggests possibly primary.
But you can stop smoking fully, and see if the need for phlebotomy is decreasing after few months.
Now since the PCV is 56, you need to undergo phlebotomy. Please meet your nearby oncologist or haematologist and proceed with same.
Also make note of what was checked in the “MPN panel”. JAK2 , MPL, CALR . Were all three genes tested? Was JAK2 testing partial? (Only V617F) - Please check these also. You could send the photo of reports here .
