Persistent hard stool despite  treatment

Hello dr, My baby is 20 months old. He has been having constipation issues for quite some time. Despite the measures I am taking, he is still passing hard, pebble-like stools, sometimes followed by softer stools. Currently, I am giving Duphalac 8ml per day from 3 months, Prunes 2 per day, Adequate water intake, Fruits and fiber-rich foods as per age. Even with this, he often strains and passes hard stools, and the area around the anus sometimes looks irritated. I am worried about whether 1. The current dose is sufficient or needs adjustment .2. Any other medicine or dietary change is required. 3. This could be related to any underlying issue Kindly guide me on the next steps.
