Please do not worry, a B12 of 86 does not mean you have pernicious anaemia. In India by far the commonest reason for low B12 is dietary, seen in vegetarians and in those who eat little milk, curd, egg or non veg food, and it also happens with long term acidity medicines, metformin or gut absorption problems. Pernicious anaemia is an autoimmune condition and is quite uncommon, and it usually shows large sized red cells with a high MCV and a low haemoglobin. Your CBC is normal and your haemoglobin of 11 is only marginally low, which goes against it. So please simply continue the B12 tablets your doctor has prescribed and take them regularly for 3 months, include milk, curd, paneer, egg and non veg if acceptable to you, and get your B12 and CBC repeated after 3 months. If your level does not improve despite regular tablets, then we can test intrinsic factor antibodies and consider B12 injections, and also check iron studies and thyroid since your haemoglobin is slightly low. Do get examined earlier if you develop tingling or numbness in the hands or feet, unsteady walking, memory difficulty, mouth ulcers or a sore tongue. Happy to review your reports if you consult me here (fee Rs 500).

You don't need to jump to pernicious anaemia. A B12 of 86 with a normal CBC is usually from diet or absorption issues, and your doctor can confirm which by reviewing your case.
Next Steps
Keep taking your prescribed B12 tablets, and ask if a methylmalonic acid or intrinsic factor antibody test is worth adding to rule it out directly.
