It appears like fungal balanitis, though exact cause could be determined by visiting a dermatologist in your area

Based on this, a combination of persistent moisture, itching, and tiny blistering lesions in the genital area points toward localized dermatological conditions, such as candidal balanitis (a fungal infection) or an acute viral skin manifestation. However, for a proper diagnosis and to differentiate between a common fungal flare-up and other infectious skin lesions, a detailed clinical examination is required.
Health Tips
To accurately evaluate the specific physical characteristics of your blistering lesions, safely review a target-healing antifungal or antiviral prescription layout in complete confidence, and relieve your anxiety, please book a direct consultation session via my Practo profile."

Penile irritation with persistent moisture, itching, and small blister-like bumps over 4 days could be due to a few things — fungal infection (Candida balanitis) is common and causes exactly this picture of moisture, itching, and skin changes. However, blister-type lesions also need to be evaluated in person to rule out other causes, as treatment differs depending on the exact diagnosis. This needs a proper physical examination by a dermatologist or urologist rather than self-treatment.
Next Steps
See a dermatologist or urologist in person as soon as possible for an examination. In the meantime, keep the area clean and completely dry, wear loose cotton underwear, and avoid any creams without medical advice. Do not delay the consultation given the blistering. Please consult me directly on Practo for a detailed evaluation.
