Pain in testicles

Hello, I am having pain in the testicles. And also pressure from back of testicles. Also in between sharp pins and needles like Pain feeling. While urinating there is no as such burning sensation. I dont know what to do. To whom I should consult ? Is this is something serious ? I took Paracetamol 650 but only little relief. What painkiller should I take.? My family physician is a Lady Doctor and I am feeling shy to consult for this problem. For the first time this has happened in 28 years. Thanks in advance

Any pain in testis which starts suddenly has to be shown to  an urologist or surgeon to rule out torsion of TESTIS which is an emergency
