Pain in neck and shoulder

When I move my head left side my left shoulder and neck starts paining and also when I lift something heavy or moderate weight up above my head shoulder starts paining and up on neck also.. i was suffering from cervical of neck before sometime but now it is almost OK but pain in left shoulder and neck is still there
