Night-time fast heartbeat, shivering and breathlessness can occur with panic attacks, but after four years we should not assume anxiety alone. Please arrange an in-person review for examination, BP, ECG and basic tests such as CBC and thyroid, and review any current anxiety treatment. Seek urgent care for chest pain, fainting, severe breathlessness or a sustained very fast/irregular pulse.
