Cracked teeth may be very difficult to diagnose. You need to understand that cracks that run mesiodistally are never seen on xrays and even cbct may not pick up cracks and sometimes extent of crack. Depends on the capability of the cbct unit itself too. In the Cbct, very careful inspection may show some bone loss near the fracture site but the radiologist needs to patience to pick this. Clinically, when the rct is done , the endodontist may be able to look for cracks.
If your dentist feels that the tooth is cracked, kindly go ahead with extraction especially since you have pain in that tooth. Often cracked teeth over time lead to severe infection. Sometimes you have to rely on clinical knowledge
