Your shoulder MRI being completely normal is actually an important clueâit strongly suggests that the shoulder joint itself is not the main source of your pain that starts in the neck and radiates to the shoulder, worsens during long drives, potholes, vibration, or sustained posture, and is absent at rest or after sleep is very typical of a cervical spine related problem cervical muscle spasm disc irritation, or nerve root irritation calcific tendinitis seen earlier on USG may have been incidental or already resolved which is why it does not appear on MRI now. when the neck is irritated pain is often felt in the collarbone shoulder, or shoulder blade area, even though the shoulder imaging is normalâthis is called referred pain. the next logical step is evaluation of the cervical spine (X-ray ± MRI if needed) and posture-related management; treating only the shoulder will not give relief if the origin is the neck.
