From your description, your symptoms are most consistent with mechanical neck pain (muscle strain/postural neck pain) rather than a serious cervical nerve problem. The absence of pain radiating to the arm, numbness, weakness, or restriction of neck movements is reassuring. A sharp-edged chain itself does not cause cervical spine disease, but if it presses on already tender neck muscles, it may make the discomfort more noticeable. Similarly, an orthopaedic mattress is unlikely to be the primary cause, although an unsuitable pillow or sleeping posture can aggravate neck pain. Prolonged mobile phone use with the neck bent forward is a very common cause of persistent neck muscle strain (“text neck”), and disturbed sleep due to caring for a toddler can further delay recovery.
Next Steps
Start a supervised cervical physiotherapy program focusing on deep neck flexor strengthening, scapular stabilisation, posture correction, and stretching of the upper trapezius and levator scapulae. Limit continuous mobile use to 20–30 minutes at a time, keep the phone at eye level, use a pillow that maintains a neutral neck position, and apply warm compresses for 15–20 minutes if muscles feel tight. If symptoms persist beyond 4–6 weeks despite these measures, or if neck pain worsens, an in-person orthopaedic evaluation is advisable, and an MRI may be considered if clinically indicated.
Health Tips
Seek urgent medical evaluation if you develop pain radiating to the arm, numbness, weakness, loss of hand grip, difficulty walking, imbalance, or bladder/bowel symptoms. If you have previous X-rays or MRI scans of your cervical spine, you may upload them or consult me online for a more specific opinion.

Use soft cervical collar for few days as it will prevent unnecessary and abnormal sudden jerky movements of neck. You can take some muscle relaxants medication. Apply hot water bag. Reduce mobile time.

This sounds most consistent with mechanical neck pain or muscle strain, especially because there is tightness and pain while bending the head, but no arm pain, numbness or weakness. Prolonged mobile-phone use with the head bent forward can aggravate the neck muscles. A chain with sharp or heavy edges may irritate the skin or press on sensitive muscles, but it usually does not damage the neck. Stop wearing it for one to two weeks and see whether symptoms improve. An orthopaedic mattress itself is unlikely to be the cause; pillow height and sleeping posture are more important.
Next Steps
Keep the phone near eye level, avoid prolonged downward bending, and change position every 20–30 minutes. Use a comfortable pillow that keeps the neck neutral. Apply a warm compress for 15–20 minutes and perform gentle neck movements, chin tucks and shoulder-blade exercises without forcing through pain. Adequate sleep and regular activity are helpful.
Health Tips
Consult an orthopaedic doctor if pain is worsening, persists beyond two to four weeks, repeatedly disturbs sleep, or limits movement. Seek urgent assessment if numbness, arm weakness, severe radiating pain, balance difficulty, fever, severe headache, or pain following an injury develops.
