For insomnia Clonazepam 0.5 or1 mg+ quitipine25 one  is best. Your  medicine  is also good. Why to tapper  nexito10  it's minimal  dose u can stop if u feel so.

QT Prolongation: Both drugs can affect QT interval, especially in higher doses or in predisposed individuals. Anticholinergic Burden: Doxepin has anticholinergic side effects, so caution in elderly or those with cardiac disease. 3. Tapering Escitalopram Should only be tapered gradually after clinical stability for 6–12 months. If tapering is attempted, doxepin may help mitigate sleep issues or anxiety during the withdrawal phase. Recommendation Co-administration is clinically acceptable in low doses under physician supervision. Regular monitoring of side effects, mood, and ECG (if cardiac risk) is advised.
