Knee pain

My father is suffering from knee pain for the past 4 years. It had reduced for 1 year in between but now it has increased again. He is diabetic and takes insulin regularly. In X-ray we can see that the gap between the joints have increased.can you suggest what he needs to do or what kind of treatment he should get. The pain is consistent and he finds difficulty standing for long time.
