Issues of low vitamins and iron level

I'm on vitamins and iron. I have attached a copy of this here. In recent times my hair has reduced drastically and I don't have a history of genetic hair loss. Can this be the reason for that. 3- 4 years back the doctor once gave me vitcofol injection for vitamins. Should I take that again ? Please give some suggestions. I also have some digestive issues like a burning feel in my stomach and bad bowel movements.

Hello, Every thing is low, iron, VitD3, b12 Vitcofol inj u can take for iron. Uprise 60,000 IU  ONE PER WEEK FOR EIGHT WEEKS FOLLOWED BY SAME ONE PER MONTH FOR SIX MONTHS AFTER BREAKFAST . VITD3 TAB NEUROKIND OD ONE AT NIGHT AFTER DINNER FOR TWO MONTHS VITB12. APPROPRIATE HEALTHY Nutritious DIET , ADD SALAD, FRUITS, DRY FRUITS. PREVENT OUTSIDE, FAST,JUNK FOOD. PLENTY OF LIQUID N WATER. SUNLIGHT ALSO EFFECTS HAIR FALL. SO USE SCARF OR HAT IN SUNLIGHT.
