The HSG needs to be repeated. If the uterine cavity is not opacified that means the dye didn't enter the uterine cavity. It has gone into the vagina because the HSG cannula got displaced. The test should be repeated.

Either go for HSG again or diagnostic hysterolaparoscopy. Hsg shows that tubes are blocked at the ends.
