First, crown-predominant thinning in a 31-year-old male is almost certainly Androgenetic Alopecia (AGA) â male pattern hair loss driven by DHT (dihydrotestosterone) acting on genetically sensitive follicles. This is the most common cause of male hair loss. Second, the oily scalp, persistent white sticky dandruff, and scalp itching together indicate Seborrheic Dermatitis â caused by Malassezia yeast overgrowth on sebaceous (oily) glands. The two conditions commonly co-exist and worsen each other, as the inflamed scalp accelerates hair thinning. Selsun (Selenium sulfide) is less effective for sebderm â switch to Ketoconazole 2% shampoo (Nizral/Keto), lather and leave on the scalp for 5 minutes before rinsing, use twice weekly. Regarding Minoxidil: occasional use does NOT work. Minoxidil must be applied daily (5% solution, twice daily) consistently for a minimum of 6â12 months to see any result â irregular use provides no benefit. The most effective medical treatment for AGA is oral Finasteride 1mg daily â it blocks DHT at the scalp and halts/reverses hair loss in most men, but needs a dermatologist's prescription. For the itching and inflammation, a topical corticosteroid scalp lotion (like Betamethasone or Clobetasol scalp solution) used once weekly under medical guidance can help settle the sebderm quickly.
