The good news is that with
B12 at 120, all your other tests (
electrolytes, CPK,
thyroid,
CRP,
HbA1c) being normal, and the nature of your symptoms â easy cramps across multiple muscle groups with post-cramp fatigue â the most likely explanation remains the B12 deficiency-related peripheral nerve injury.
Vitamin B12 deficiency at severe levels (like 120) damages the myelin sheath around nerves, which affects how muscles receive signals. The critical thing to understand is that nerve recovery is slow â it typically takes 3-6 months of consistent supplementation before significant improvement is noticed, and sometimes up to 12 months for full recovery. Three weekly loading injections is just the first phase. One additional thing worth checking: serum magnesium specifically. The standard 'electrolytes' panel often does not include magnesium, and magnesium deficiency is one of the most common and under-tested causes of easy muscle cramping that persists despite other treatment.
Next Steps
Get serum magnesium checked â this is often missed in routine electrolyte panels and is a very common cause of persistent cramps. Also recheck your current
B12 level to see where it stands after treatment. Continue B12 supplementation â monthly injections or daily Methylcobalamin 1500 mcg orally. If magnesium is low, take Magnesium glycinate or Magnesium citrate 200-400 mg daily. Ask your doctor about optimising the Gabapentin dose if neuropathic symptoms persist. Be patient â full nerve recovery at your B12 level takes several months. Please consult me directly on Practo for a detailed evaluation.
