Forgetting things

I got to know that I had ADHD since my fifth grade.. And since my childhood... I'm very forgetful about things... And recently I've observed that this problem has become more... I'm not able to concentrate on stuff... And I've a lot going on with my family and relationship... I've terrible mood swings one minute I'm sad and the other I'm happy...its too much to take... I can't even remember the happy moments I spent till now... And if I've to continue saying there are a lot of things... I've anxiety... I feel that pain kind of thing in my chest and stomach when something unfortunate happens... All of these things are affecting my grades which I don't want to... Please give me a solution for this...
