It doesn't look all that bad. But since you have symptoms it is better to consult with a General Surgeon in your area. He may advise removal of at least one nail completely.

I am seeing longitudinal parallel white ridges over the nail.. this might be fungal infection of the nails or even protein deficiency with liver disease. Kindly consult to me for proper investigations, diagnosis and legal prescription. Thank you
