Fissure due to narrow anal sphincter

How to heal the fissure and normalise the anal sphincter without surgery? Taking stool softener and isabgol daily but no improvement. No ointment or sitz bath helped. Don't want to do surgery as I can't take break from work and I have a desk job.

Isabgol usually increases stool bulk and so doesn't help Laxative needs to be changed. Ointments need to be changed. Sits bath is essential. Diet needs to be changed.
