Doubts regarding CBC report

2026-05-21 10:25:23
My CBC shows microcytic hypochromic anemia with a high RBC count and a borderline Mentzer Index of 13.8. My ferritin is 7.88 which is functionally low. The pattern is inconclusive between iron deficiency and beta thalassemia trait. Should I get an HPLC Hemoglobin Electrophoresis to rule out thalassemia before continuing iron supplementation?

You have already asked a similar question. As discussed, yes — with a borderline Mentzer Index of 13.8 and microcytic hypochromic anaemia with high-normal RBC count and very low ferritin, getting an HPLC (Haemoglobin Electrophoresis) is absolutely the right next step before assuming it is purely iron deficiency. The two conditions — iron deficiency and beta thalassemia trait — can coexist, and distinguishing them correctly is important for the right treatment.
Next Steps
Please consult me directly on Practo for a detailed evaluation — I can guide you on the HPLC test and interpret the results in context of your full picture.

Ferritin of 7.88 strongly suggests iron deficiency, and iron supplementation should be continued unless advised otherwise by your doctor. However, a high RBC count with microcytosis can also be seen in beta-thalassemia trait. HPLC/Hemoglobin electrophoresis is a reasonable next step, but it is often best performed after correcting iron deficiency, as low iron can affect HbA2 levels and mask thalassemia trait. Kindly consult so that we can discuss further treatment plans.
