Good question.
So here pain and sensitivity are two terms to be taken differently.
Considering your thing regarding cavities and fillings, i consider it by default that its not a deep cavity.
So we a dentist cuts through enamel and dentin, you would feel mild sensitivity which can be swallowed as pain by some patients, mild sensitivity by some patients. So basically it depends on the pain threshold of the patient on how he or she feels.
Secondly, if the dentist is too rough or fast in giving motion to the bur, obviously more sensitivity can be felt which patient receives as pain.
Moral- if one cant tolerate the sensitivity, its the dentist's job to give anesthesia and carry on.
