Can severe vitamin d3 deficiency (5.3ng/mL) cause delayed periods? I have also taken calcium citrate tablets and multivitamin tablets along with aceclofenac ip 100mg for 4 days.

Your Vit D3 level is unusually low and needs treatment per se. Delayed periods have many causes. Vit D deficiency is not a very common cause.
