Hi , after looking at CT scan of PNS , there is deviation of your nose bone and cyst in maxillary sinus, before going to advise for any surgical option I need to know about your symptoms , is it really disturbing ? Then we can think of further option wether it is medically manageable or surgery is required or not

Depends on Ur symptoms. U have very severe  dns and spurr to the RT . Which may require surgery. Mucosal retention Cyst is not that significant. But if u r going for dns surgery that time cyst surgery through endoscopy can also be done in one go.
