In 2nd tri constipation is common due to hormonal imbalance and pressure. Cremaffin is safe for shorter duration.
Next Steps
do walk after eating. add fibers in your diet fluid intake use isapgul(psyllum husk) it is safe and without side effect

Yes Cremaffin is safe in pregnancy for short-term use. Since lactulose alone isn't working after 2 days, adding Cremaffin Plus (1-2 tbsp at night) is a good next step. Also try warm prune juice or warm lemon water in the morning. Isabgol (psyllium husk) sachets daily also help a lot during pregnancy.
Next Steps
Give Cremaffin Plus 2 tbsp at night for 2-3 days. Add Isabgol sachets twice daily with water. Short walks after meals help. If no relief in 48 hours, consult her gynecologist. Consult me on Practo for more guidance.

Initially a suppository and enema can be put for passage of long standing residual load then along with diet management, gradually dose adjustment of laxatives like cremaffin or higher dose lactulose can be taken safely in pregnancy. Apart from that, diet management is vital, add loads of probiotics like curd and fermented foods to your diet. Observe your diet for foods causing such constipation anf kindly connect for further assistance.

Constipation is common during pregnancy. Cremaffin may be used during pregnancy when advised by her obstetrician, but avoid repeatedly increasing laxative doses without medical supervision. Since lactulose has not helped despite adequate fluids and fibre, she should consult her obstetrician for examination and an appropriate alternative, such as a bulk-forming agent or polyethylene glycol (PEG/macrogol). Seek urgent medical attention if she has severe abdominal pain, vomiting, marked abdominal distension, bleeding, or inability to pass gas.
