ME/CFS is a real condition, but it is a diagnosis of exclusion: it requires at least six months of disabling fatigue with post-exertional malaise (feeling much worse for a day or more after even mild activity), unrefreshing sleep and cognitive "brain fog", once other causes have been ruled out. Your eight months of fatigue with slowed thinking fits the pattern, but common treatable causes - anaemia, iron and
vitamin B12 or D deficiency,
thyroid disease,
diabetes, sleep apnoea, post-viral states and depression - must first be excluded properly. Antidepressants alone are often not the answer if the underlying problem has not been mapped out.
Next Steps
Collect all your reports in one file and stay with one general physician as your main doctor rather than moving between many. A reasonable baseline is CBC, ESR/
CRP,
ferritin,
vitamin B12 and D,
TSH,
HbA1c,
LFT, RFT,
electrolytes, coeliac screen and, if you snore or wake unrefreshed, a sleep study. Use pacing - plan activity within your energy limit and avoid push-crash cycles - along with a fixed sleep schedule and a symptom diary. Please continue psychiatric follow-up rather than stopping medicines abruptly. You are welcome to consult me on Practo or at Synergy Miraj to review the reports together.
Health Tips
Do not stop or change psychiatric medicines on your own; discuss tapering with your treating doctor. Seek urgent care for chest pain, breathlessness, fainting, marked weight loss or any thoughts of self-harm.
