Radiologist: There is a large midline complex mass lesion in the pelvis showing regions of T2 hyperintensity and intermediate signal intensity, with heterogeneous irregular frond like T2 hypointense regions in between. Part of lesions appear hyperintense on T1 and T1 fat saturated sequences. Patchy diffusion restriction noted in the T2 hypointense region. Approximate measurement is 9.1 x 4.7 x 8 cm. Post-contrast study shows mild septal enhancement, with subtle patchy enhancement noted in T2 hypointense region The above lesion is adherent to the uterus. Ovaries are not seen separately. Impressions: Pelvic mass is concern for endometrioma, as detailed above, with mitotic features. D/D mucinous cystadenocarcinoma. Ovaries are not seen separately.
