Need clinical examination of patient and need to see MRI Films to see grade of neural compression If compression is more definately surgery is needed. Spine surgery is a major surgery Avoid cervical traction for timebeing
Next Steps
kindly consult orthopaedic spine surgeon near you as patient is unable to stand freely

Need to examine and differentiate between whether the pain is because of diabetes or spinal cord compression. Once we diagnose we will have to see MRI films to confirm need for surgery. If it requires, then definitely yes. May be traction and physiotherapy wont improve his condition
