Breathlessness while eating

Every now and then I feel short of breath while eating. It comes and goes. What it could be? And I also feel chest pain it also comes and goes. I've been having chest pain like this for 4 years now it lasts for a week or two and then goes away and then I feel it again after sometime. What it could be?
