Your blood pressure reading is high.Accordingly your doctor has started to treat you with a combination antihypertensive. The second doctor you have consulted has prescribed the same medicine with a higher dose component. You have to use one of them daily, not both.Proper followup with your doctor is required for the management of hypertension.

According to your bp both your doctor prescribed antihypertensive meds . Telmisartan40 & ctd 6.25 ( one tab helps to lose Na+ and one is to help increase urine frequency ,both together help maintain your BP) And other tab telma H is a combination of  telmasartan+hydrochlorothiazide so this tab will work as a same like previous one so take only one prescription medicine .
Next Steps
low salt intake food and add more fiber in your diet, do physical activities, maintain healthy lifestyle

I don't like to comment on the treatment started by other Doco, especially for diseases like Hypertension and Diabetes. There are no laid down guidelines about what is "one -two- three -four" in the treatment schedule. The proof of the pudding lies in eating it. Wait for 15 days and get the BP checked from the same Physician. Either he will continue with the same medicine or he will change it.
