Bone graft for implant and extraction

I have a tooth (46) which is hurting for more than 2 years. Tapping and biting pressure on it. Went to Endo. CBCT scan and xrays are clear. Did first root canal in 2020 and no relief. Same symptoms. Saw another Endo and did partial distal root retreat last year and no changes. Same pain/symptoms. Endo told me to extract it as nothing worked. But he reopened the tooth to check one last time before sending be to surgeon for ext. When he reopened, mesial canal filling was wet, stinking and blood color changes like purplish. But no pus and he didn't say if it's infected. He placed calcium hydroxide inside and it's been 41 days now. No relief. So he told me to go for ext now. Taken CBCT again and nothing found. But they told me to extract it,since no relief from pain. My primary pain is in lower jaw under ear and that's how pain started. If I extract it,do I need to do immediate bone graft only? I want to do implant later. Can I do bone graft after few years of extraction? Help.

You should take a second opinion from an endodontist before going for an extraction. If an extraction is planned then an immediate implant can be placed following extraction or a delayed one, after the extraction site is healed completely. In both the cases bone grafting may or may not be required. That depends upon the amount of available bone at the time of implant placement.

The root canal treatment should be checked again by a consultant Endodontist. Extraction of the tooth is the last choice. Proper Re evaluation of obturation and post obturation in the Root Canal Treatment Procedure must be done. Cleaning and Shaping of the canals should be reevaluated. Proper Antibiotic regime should be prescribed and you need to follow the course thoroughly
Next Steps
Consult an Endodontist for second opinion before going for extraction
Health Tips
After extraction, tooth sockets heal naturally by physiologic process and there may not be any need for grafts.

There might be a fracture which may not be visible on CBCt sometimes. You can go for extraction and immediate implant placement . You can also get extraction done and get the implant after three months . No need for a graft . !!!
